Furthermore, I understand that any misrepresentation, falsification, or material omission of information in the application may result in my failure to receive an offer or, if I have been hired, my immediate dismissal from employment.

Prairiewoods is a sponsored ministry of Franciscan Sisters of Perpetual Adoration (FSPA). I understand that all offers of employment are contingent upon satisfactory proof of my identity and legal authority to work in the U.S. Offers of employment are also contingent on FSPA's receipt of post-offer satisfactory responses to reference requests and satisfactory completion of a physical exam, required tests and background checks.

If employed by FSPA, your employment is at will. This means that your terms and conditions of employment may be changed with or without cause and with or without notice, including but not limited to termination, demotion, promotion, transfer, compensation, benefits, duties and location of work. Your status as an at will employee cannot be changed except through a written agreement signed by the President of FSPA.

I authorize FSPA to investigate my responses on this application and contact any or all of my former employers or any individuals familiar with my employment background for the purpose of verifying any information I have provided and/or for the purpose of obtaining any information about my employment. I release all parties and persons from any and all liability for any damages that may result from furnishing such information and release and hold harmless the Franciscan Sisters of Perpetual Adoration from any liability for such inquiries.
